Our Team: The Arizona Reach program is a 24/7 service that coordinates acute medical care transfers out of IHS, PL 93-638, and critical access hospitals across Arizona. AZ REACH streamlines the transfer process by facilitating calls for placement, connecting practitioners, and following through on placement progress, allowing referring facility team members to focus on care. This position requires clinical proficiency and a strong dedication to addressing healthcare disparities, overcoming access barriers, and advancing equity in care. The Case Manager will work closely with care teams and key stakeholders to ensure AZ REACH patients receive high-quality support, with a particular focus on ensuring efficient and seamless transition of care during the discharge process and continuum of care.

Duties & Responsibilities

  • Fosters collaborative relationships with care teams across the healthcare continuum, including physicians and payers. Collects and disseminates relevant, timely information to the patient's healthcare team to ensure comprehensive care and to meet utilization and regulatory requirements.
  • Responsible for assisting care teams in connecting patients to appropriate post-discharge services and coordinating specialty referrals with outpatient care teams.
  • Maintains knowledge of Medicare, Medicaid, Purchased Referred Care (PRC), and other benefit programs to support discharge planning and/ or ongoing care. Stays informed about community resources available in the patients area to address their specific needs and access to care.
  • Monitors and ensures comprehensive documentation of the patient's care plan, progress, and all communications with involved parties.
  • May provide supervision and training to other staff members and performs assigned duties and tasks to support the department's needs. Provide on-call support during scheduled rotations or as required.

Minimum Qualifications

  • Bachelor's degree or equivalent advanced learning attained through professional level experience required.
  • Minimum of 3 years of relevant work experience, or equivalent combination of education and work experience.

Preferred Qualifications

  • Bachelor's degree in Nursing is strongly preferred.
  • Case Management experience strongly preferred.

Notice of Availability of the Annual Security and Fire Safety Report

In compliance with the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (Clery Act), each year the University of Arizona releases an Annual Security Report (ASR) for each of the University's campuses. These reports disclose information including Clery crime statistics for the previous three calendar years and policies, procedures, and programs the University uses to keep students and employees safe, including how to report crimes or other emergencies and resources for crime victims.
